Does testicular cancer impact sexuality and fertility? Here’s the truth

Testicular cancer is highly curable, especially in stage 1, where 99 percent of patients are cured when the disease is limited to one testis only. The specific treatment received, the stage of cancer, and various other factors contribute to the potential impact on sexuality and fertility. Even in higher stages, the cure rate remains high at around 95 percent, emphasizing the importance of discussing sexual life and fertility. Various studies have explored this topic, revealing that testicular cancer can lead to varying degrees of sexual dysfunction. Research published in the journal Cancer demonstrated a 50 percent pregnancy rate among female partners of testicular cancer patients who had undergone sperm cryopreservation prior to chemotherapy.

Testicular cancer can affect a person's sex life and ability to have children, but it should be kept in mind that the extent of these effects can vary from person to person. The specific treatment received, the stage of cancer, and various other factors contribute to the potential impact on sexuality and fertility.

Treatment and sexual dysfunction

Even in higher stages, the cure rate remains high at around 95 percent, emphasizing the importance of discussing sexual life and fertility. Various studies have explored this topic, revealing that testicular cancer can lead to varying degrees of sexual dysfunction. Factors such as the treatment modality (surgery, radiation, or chemotherapy), psychological aspects, and age at diagnosis play significant roles, says Dr Dinesh Singh, Chairman Radiation Oncology, Andromeda Cancer Hospital, Sonipat.

If a person undergoes surgery involving abdominal nodes, it may lead to problems with erection and ejaculatory function. Dry ejaculation or reverse ejaculation, where semen is directed back into the bladder, resulting in its release during urination, can occur.

For individuals undergoing chemotherapy, the use of barrier contraception methods is recommended. Some patients may experience body image issues and fluctuations in libido, says Singh.

Fertility considerations

While the removal of one testicle typically does not impair fertility, especially if the remaining testicle is healthy, bilateral testicular involvement or additional treatments like radiation or chemotherapy can affect fertility, says Singh. In such cases, discussing fertility preservation options with a healthcare team is advisable. Sperm banking, for instance, offers a means to safeguard fertility before undergoing treatment.

Coping strategies

It's essential for patients and their partners to openly discuss any concerns they may have regarding sexuality and fertility. Many issues can be resolved through dialogue and counseling. Participating in support groups where individuals can connect with others who have faced similar situations can also be beneficial. Sharing success stories of patients who have undergone treatment and are leading fulfilling sexual lives and fathering children can provide valuable morale and confidence boosts.

Embracing life after testicular cancer

Testicular cancer should be approached with a proactive mindset, aiming to maintain as normal a life as possible, says Singh. Remaining physically active through exercise, avoiding tobacco and excessive alcohol consumption, maintaining a healthy weight, adopting a balanced diet, practicing yoga and Pranayama, and engaging in other stress-reducing activities can all contribute to overall well-being and recovery.
