Hydrocele in adult men: What are the signs and how to prevent it from advancing

In the remote stretches of Uttar Pradesh, Narender, a 42-year-old farmer, noticed a disturbing change—a painful swelling in his scrotum that grew progressively heavier and more uncomfortable each day. Embarrassed and fearful of social ostracism, Narender initially chose silence over seeking medical help. His story illustrates the profound impact of hydrocele on individuals' lives—disrupting their health, work, and social life.Hydrocele manifests as a swelling caused by fluid accumulation in the scrotum, resulting from lymphatic filariasis , a parasitic infection transmitted by mosquitoes. Patients may experience a sensation of heaviness or discomfort in the scrotum which can lead to fluid retention. In some cases, particularly when the hydrocele is large, individuals may experience difficulty walking or performing daily activities. According to experts, at least two in every three people who have Filarial Hydrocele do not have severe symptoms, which leads to a quick weakening of the immune system. Some people do experience lymphedema (fluid buildup in the lymphatic system), swelling, edema (swelling in the arms and legs), and fever in rare cases. This condition is particularly prevalent in India’s tropical climate, which is conducive to the Culex mosquitoes that spread this disease. While hydrocele is primarily a physical health issue, it carries significant social stigma that can lead to psychological distress and isolation for those affected, making it more than just a medical condition.India bears a significant burden of hydrocele, with over 1.26 lakh reported cases across the country as of 2023. The prevalence of this condition underscores the urgent need for effective public health interventions and community support systems.Managing hydrocele in India poses medical and social challenges, yet efforts are underway to improve treatment and prevention. Early detection and access to healthcare is a key area of focus. Many men, especially in rural areas, struggle to access effective medical care, hence more initiatives should be developed to address this issue and spread sensitization to overcome fear of surgery and build trust in medical interventions.Socially, the stigma surrounding hydrocele is profound, men suffering from the condition often face ridicule and isolation, driven by widespread misconceptions about the disease’s origins and implications. This stigma not only discourages individuals from seeking the necessary medical help but also affects their mental health and social standing, leading to temporary discomfort-disability, isolation, and economic hardship as their ability to work is compromised. Moreover, the intersection of cultural beliefs and lack of education about lymphatic filariasis exacerbates these challenges. Communities often lack basic health education, which is crucial for understanding symptoms and recognizing the importance of seeking medical treatment. This gap in knowledge fuels ongoing cycles of stigma and neglect, hindering effective and timely public health interventions.Preventive measures to address the disease at its early stages include maintaining proper sanitation and hygiene practices, specifically in the genital area, and taking steps to protect the scrotum from injury. However, for complete treatment of the disease, early intervention is recommended. Hydrocelectomy, a minor surgical procedure, is often suggested. This surgery involves repairing and removing the hydrocele. In a standard hydrocelectomy, the surgeon makes a small incision in the scrotum and drains the hydrocele using suction. Surgery offers the most long-lasting repair with a significantly lower hydrocele recurrence rate.Currently, India is implementing Mass Drug Administration (MDA) and Morbidity Management and Disability Prevention (MMDP) as key components for the elimination of Lymphatic filariasis. These programs aim to educate the public about the symptoms, prevention, and available treatments for hydrocele. However, it is crucial for the broader community to join forces to raise awareness about the social stigma surrounding this condition and to guide individuals towards appropriate treatment and solutions. By fostering understanding and empathy within communities, these efforts help dispel the myths associated with the condition, making it easier for individuals like Narender to seek help without fear of judgment.Hydrocele is a testament to the broader challenges of public health in India—challenges that require a collective response involving government action, community engagement, and individual empowerment. As we observe International Men's Health Week, let us reflect on the strides made and the journey ahead. It is a path that demands continued commitment to medical interventions and a profound shift in societal attitudes towards health and dignity.
